Wolfpack One Card Request Form

Wolfpack One ID cards are made at the Wolfpack One Card office, located in Talley Student Union, from 8 am to 5pm Monday – Friday. Currently registered students and NC State employee can get a campus ID card by showing a photo ID at the office.

For Visitors, Contractors, or Affiliates, card request forms must be completed by a sponsoring member of NC State faculty or staff before a card can be issued.

Please complete the form below before coming to the office to collect your card(s).

Please note that fraudulent requests may result in disciplinary action.
